Highlights of the Care Coordinator| Lead Home Health Aide position:
• Coordinate caregiver schedules and resident service requests
• Ensure residents receive exceptional care and personalized support
• Mentor, train, and support Home Health Aides and caregivers
• Assist with staffing, scheduling, documentation, billing support, and service coordination
• Conduct resident satisfaction follow-ups and quality assurance visits
• Help maintain continuity of care and service excellence
• Provide direct resident support when needed, including:
What We're Looking For
• Home Health Aide certification or caregiving experience preferred
• Previous scheduling, staffing coordination, or leadership experience preferred
• Experience working with older adults strongly preferred
• Strong organizational, communication, and customer service skills
• Ability to manage multiple priorities in a fast-paced environment
• Valid driver's license and reliable transportation required
• Positive attitude, strong work ethic, and passion for serving seniors
