Care Coordinator – Caring Senior Service, South Carolina (Horry County) 

A Care Coordinator manages client care services and acts as the bridge between clients, caregivers, and the office team to ensure high-quality in-home support.

Core Responsibilities

1. Client Care Management

  • Conduct initial phone call for new clients (needs, preferences, safety concerns)
  • Monitor client satisfaction and adjust services as needed
  • Perform periodic check-ins (phone or in-home visits)

2. Caregiver Coordination

  • Match caregivers to clients based on skills, personality, and availability
  • Communicate schedules and care expectations
  • Address caregiver concerns or performance issues
  • Support training or onboarding when needed

3. Scheduling & Logistics

  • Coordinate and maintain caregiver schedules
  • Handle last-minute call-offs and find replacements quickly
  • Ensure all shifts are properly staffed

4. Communication Hub

  • Act as the main point of contact for clients and families
  • Collaborate with healthcare professionals (nurse)
  • Relay important updates between caregivers and management

5. Documentation & Compliance

  • Maintain accurate client records and care notes
  • Ensure services meet state regulations and company policies
  • Track incidents, changes in condition, and service adjustments

6. Problem Solving & Crisis Management

  • Respond to emergencies or urgent client needs
  • Resolve conflicts between clients and caregivers
  • Adapt care plans quickly when situations change

Required Skills & Qualifications

  • Experience in healthcare, home care, or social services
  • Strong organizational and multitasking abilities
  • Excellent communication and interpersonal skills
  • Ability to handle stressful situations calmly
  • Basic computer skills (scheduling systems, documentation)
  • Often requires a valid driver's license (for home visits)
