What You'll Do
As a Care Coordinator, you'll be the connection between families, providers, and services - ensuring members receive the highest quality care.
- Manage and support a caseload of DDD members
- Conduct home visits and build strong relationships with families
- Coordinate Attendant Care, Habilitation, and Respite services
- Advocate for member needs and ensure services align with PCSPs
- Collaborate with DSPs, families, and support teams
- Monitor service utilization, compliance, and outcomes
What We're Looking For
We're seeking motivated professionals with a heart for service and experience in:
- HCBS, DDD, Case Management, or Care Coordination
- Group Home Management or Direct Support Professional (DSP) roles
You'll thrive in this role if you:
- Have strong communication & relationship-building skills
- Are organized, detail-oriented, and solutions-driven
- Understand Arizona DDD systems and service delivery
- Are passionate about helping individuals with developmental disabilities live meaningful lives
Requirements
- Experience in HCBS / DDD / Case Management / DSP roles
- Knowledge of Arizona DDD programs and services
- HIPAA compliance and confidentiality awareness
- Strong computer skills (Google Workspace, Apple preferred)
- Valid Level 1 Fingerprint Clearance Card
- Valid driver's license & reliable transportation
- Ability to pass APS & CPS registry checks
