Overview:
The Care Coordinator is a Registered Nurse who is assigned to a specific clinical care area, facilitating the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator will focus on anticipation, integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients and to optimize clinical and resource outcomes. The Care Coordinator collaborates with the interdisciplinary team in assessing patient needs and developing the plan of care. The Care Coordinator will also act as an advocate for the patient, physician and hospital relative to compliance with federal, state and third party utilization management regulatory requirements.
What You’ll Do
- Serves as a central communication hub between hospitalists, EDs, bed placement/transfer center, nursing, and ancillary services
- Assists with daily census management and throughput optimization
- Coordinates ED admissions and patient placement, including acceptance and service assignment
- Supports real-time patient flow from ED to inpatient units
- Coordinates care for hospitalized patients to ensure progression from admission through discharge
- Communicates patient status and care priorities across interdisciplinary teams (MDs, APPs, Nursing)
- Promotes adherence to clinical pathways, workflows, and regulatory standards
- Identifies system issues impacting patient flow and supports operational improvement efforts
- This role is not a Case Manager position. The focus is on operational coordination and patient flow optimization.

What We’re Looking For
- Experience in one or more of the following preferred:
- Emergency department workflows
- Hospital throughput or bed management
- Transfer center or admissions coordination
- Experience with Epic or similar EMR systems preferred
- Clinical triage and medical decision support
- Strong understanding of hospital throughput and patient flow
- Excellent interdisciplinary communication skills
- Ability to problem-solve in real time and escalate barriers appropriately
- Strong organizational and prioritization skills
