Casa de la Familia provides mental health services to individuals and families affected by trauma, anxiety, depression, and other behavioral health conditions. We partner with healthcare providers, social service agencies, schools, community organizations, and public agencies to improve access to care for underserved communities. Position Summary

Casa de la Familia is seeking a Bilingual Care Coordinator to support members enrolled in the Enhanced Care Management (ECM) program through CalOptima. This position serves as a key point of contact for members and assists with outreach, engagement, appointment scheduling, care coordination, resource linkage, and documentation activities.

The Care Coordinator works closely with members, healthcare providers, community organizations, and internal care teams to help individuals access medical care, behavioral health services, housing resources, food assistance, transportation, and other community-based supports.

This position is ideal for candidates who enjoy helping people, communicating by phone, solving problems, and connecting individuals with resources that improve health and quality of life.

Responsibilities

  • Conduct outbound and inbound calls with CalOptima ECM members.
  • Engage members and assist with ECM enrollment and participation.
  • Schedule appointments and coordinate services with healthcare providers and community agencies.
  • Assist members with accessing medical, behavioral health, and social service resources.
  • Maintain accurate and timely documentation of member interactions.
  • Monitor assigned outreach activities and complete follow-up tasks.
  • Verify and update member demographic and contact information.
  • Support care managers with care coordination activities and member follow-up.
  • Communicate professionally with healthcare providers, health plans, community organizations, and referral sources.
  • Assist with obtaining records, authorizations, and required program documentation.
  • Track referrals and resource connections for assigned members.
  • Participate in team meetings, trainings, and quality improvement activities.
  • Maintain compliance with HIPAA and confidentiality requirements.

Qualifications

  • Bilingual in English and Spanish required.
  • High school diploma or equivalent required.
  • Associate's degree or coursework in psychology, sociology, social work, public health, healthcare administration, or a related field preferred.
  • Minimum one year of experience in healthcare, social services, customer service, care coordination, case management support, community outreach, or a related field preferred.
  • Strong verbal communication and customer service skills.
  • Comfortable speaking with members by phone throughout the day.
  • Strong organizational and time management skills.
  • Ability to manage multiple tasks and priorities.
  • Proficiency with Microsoft Office, including Outlook, Word, and Excel.
  • Experience working with underserved populations preferred.
  • Knowledge of community resources and healthcare systems preferred.
  • Ability to work independently and as part of a multidisciplinary team.

Preferred Experience

  • Medi-Cal, CalOptima, or managed care experience.
  • Enhanced Care Management (ECM) experience.
  • Community Health Worker (CHW), care coordination, case management, or member services experience.
  • Experience documenting services in electronic health record systems.
  • Experience working in behavioral health, social services, healthcare, or community-based organizations.
