Position Overview:

The Care Coordinator supports children with chronic medical or behavioral health conditions by connecting them to the services they need to improve their overall health and well-being. This role works primarily in the field and as part of a team to assess client needs, develop person-centered care plans, and coordinate services. The position plays a key role in helping clients and families navigate healthcare and community systems while promoting positive health outcomes. Care Coordinators also monitor progress, provide follow-up support, and help ensure continuity of care.

 

This position is located in Staten Island, New York.

   

 

Key Responsibilities:
  • Conduct client assessments to identify needs and develop care plans
  • Coordinate and link clients to mental health, medical, and social services
  • Maintain accurate documentation, case notes, and service records
  • Monitor client progress and update care plans as needed
  • Collaborate with providers, families, and community agencies
  • Provide ongoing support, follow-up, and crisis intervention when necessary
Requirements:
  • Bachelor’s degree required in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, or speech and hearing
  • Must have at least two years of experience providing direct services or case management services to mentally disabled, chronically ill, or homeless children with complex needs
  • Intermediate computer proficiency required
  • Fluency in a second language such as Spanish, Russian, or Cantonese/Mandarin is a plus
Pay Range: USD $24.31 - USD $24.31 /Hr.
