North County Serenity House (NCSH), a residential program of HealthRIGHT 360, was founded in 1966 to provide substance use disorder services in the community. NCSH provides a gender-responsive and trauma-informed environment using evidence-based and best practices that recognize and account for the role that trauma frequently plays in substance use and criminal histories of women.

For clients with co-occurring mental illness, we provide integrated substance use and mental health services that treat both conditions as primary. Our residential facility serves up to 120 women with capacity for up to 20 children under 5 years of age seeking recovery from substance use disorders.

The Care Coordinator Temporary Special Projects (SP) provides comprehensive support and advocacy for our clients at NCSH. They assess the needs and strengths of our clients and their children to promote family health and recovery. They provide case management in both individual and group settings, deliver program-specific curricula, and assist clients with children in navigating systems of care within a supportive environment.

Care Coordinator II SP acts as a liaison between the program and Child Welfare Services to ensure comprehensive care for our clients.

Key Responsibilities

Direct Client Interaction and Support

• Facilitate individual sessions with each caseload client who is involved with Child Welfare Services (CWS). • Provide learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals. • Perform crisis intervention and communicate with the treatment team as unforeseen situations arise. • Facilitate groups assessments and treatment plans. • Collaborate with each caseload client and other available internal and external resources to develop and maintain treatment plans, transition plans, progress notes, and appropriate updates in support of the health and recovery needs of the client.

Proactively link clients to both internal and external resources based on their treatment needs and follow up on the progress status.

Attend Team Decision Making Meetings when invited by Child Welfare Services and maintain regular contact with CWS workers to allow for advocacy for each client.

Facilitate monthly case conferences for clients who are involved with Child Welfare Services, which include all parties involved in the clients case.

Attend collaborative effort meetings, such as with Child Family Team sessions. These sessions can include members of the childs school district, CWS, social workers, caregivers, non-custodial parents.

Documentation and Compliance

• Document client updates, incidents, changes in visitations, and placement in the clients file. • Properly document all individual and group counseling sessions and complete the discharge paperwork process and required agency assessments in a timely manner. • Maintain accurate records by entering data into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements. • Ensure that documentation complies with agency HIPAA, 42CFR, and funder standards. • Properly document all services provided and any other documentation needed in the clients record and ensure that the golden thread is documented throughout the chart.

Administrative

• Report to other referring agencies weekly, including CWS and Dependency Drug Court. • Plan and facilitate client celebrations and special events. • May work weekends and holidays as needed.

Qualifications

• Education: High school diploma or equivalent required. Bachelors degree in Health and Human Services, Social Work, Psychology, or other related field preferred. • Certification or Licensure: Registration or Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP). • Valid California Drivers License and access to registered and insured transportation. • Experience: At least one year of case management experience, preferably with similar populations. • Experience carrying a caseload. • Must complete background check and livescan clearance.
