QUALIFICATIONS

Preferred

  • Bachelor's degree in a healthcare-related field
  • Relevant certifications in care coordination or case management
  • Strong communication, organizational, and interpersonal skills
  • Ability to manage multiple priorities in a fast-paced healthcare environment
  • Proficiency with electronic health records (EHR) and Microsoft Office applications is a plus
RESPONSIBILITIES
  • Assist patients with navigating healthcare services and available resources.
  • Coordinate individualized care plans with physicians, nurses, and other healthcare providers.
  • Evaluate patient needs and monitor health outcomes.
  • Communicate effectively with patients, families, and interdisciplinary care teams.
  • Document patient interactions, care plans, and progress notes accurately and timely.
  • Support continuity of care while maintaining confidentiality and compliance with healthcare regulations.
  • Perform additional duties as assigned.
