About the Role:

We are seeking a dynamic and relationship-driven Care Liaison / Nurse Liaison to join our team! In this role, you will serve as the vital connection between hospitals, healthcare providers, and our skilled nursing facilities, helping patients transition smoothly into the next level of care.

Major Responsibilities:

  • Coordinate and evaluate patient admissions across multiple care centers
  • Build and maintain relationships with hospitals, physicians, insurers, and community partners
  • Conduct on-site hospital visits to educate referral sources and convert referrals to admissions
  • Complete pre-admission assessments, including clinical, financial, and insurance review
  • Serve as a trusted liaison between patients, families, and care teams
  • Educate patients and families on rehabilitation services, expectations, and care options
  • Collaborate with leadership to meet and exceed census and growth goals
  • Manage referral pipelines and maintain a target list of key contacts and decision-makers
  • Troubleshoot service concerns and ensure high referral satisfaction and retention
  • Participate in community outreach, networking events, and business development initiatives
  • Stay current on Medicare, Medicaid, and insurance guidelines
  • This role requires travel between hospitals, healthcare facilities, and our centers. You will work closely with patients, families, and interdisciplinary teams in a fast-paced, ever-changing healthcare environment.

 

Minimum Qualifications:

  • Nursing degree (preferred) or relevant healthcare/business background
  • Current active and unencumbered state nursing license (if applicable)
  • Minimum 3 years of experience in business development, admissions, or healthcare marketing
  • Experience with managed care, insurance, or payer sources preferred
  • Strong understanding of skilled nursing or post-acute care
