Director of Case Management

Position Overview:
We are seeking an experienced Director of Case Management to oversee care facilitation, patient flow, and hospital continuum management. This role is responsible for leading Case Management, Social Work, Utilization Management, and Post-Acute Services Coordination to ensure seamless patient transitions and optimal care progression.

Key Responsibilities:

• Develop and implement a hospital-wide Case Management program to enhance patient care coordination.
• Oversee staff roles, care progression, and physician collaboration to improve patient outcomes.
• Ensure compliance with utilization management, social work, and post-acute services processes.
• Drive strategic initiatives to enhance patient flow, discharge planning, and care transitions.

Qualifications:

Required:

• Active state RN license.
• 5+ years of case management experience in a healthcare setting.
• Proven leadership, communication, and customer relations skills.

Preferred:

• Masters degree (preferred).
• 3-5 years of supervisory experience.
• Formal case management training.
