POSITION SUMMARY:
The Care Manager (Adult Services) works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered trauma informed, individualized focused approach utilizing evidenced-based practices delivered to fidelity. These services are delivered in multiple types of settings including field-based services in the individual’s home or other identified locations as well as clinic-based services. Individuals served may have multi-system involvement such is Criminal Justice, Child and Adult Welfare, Veterans, Homelessness, etc.
PROGRAM DESCRIPTIONS:
Adult Services – Outpatient – This position works in the Outpatient programs and among the various Adult facility locations. Services in this program include individual and group outpatient services for adults which includes Crisis Services, Medication Management, Psychosocial Rehabilitation, Case Management, Skills Training and Complex Services. Other programs include Substance Use Disorder and Criminal Justice.
Adult Crisis Services – This position works in the Crisis Unit or the Josephine Recovery Center. Services in the unit include emergency and planned crisis care for adults ages 18 and over who have a behavioral health diagnosis or need. The program provides meals, medication monitoring, supervised recreation, skills training, community referrals, and 24-hour supervision in a secure environment. The Josephine Recovery Center is not a secure environment, but provides the same services as The Crisis Unit. In addition to these sites CHCS operates a Mobile Crisis Outreach Team (MCOT), a resource designed to bring a crisis worker into the community to provide a face-to-face assessment and intervention, follow-up and relapse prevention services.
ESSENTIAL FUNCTIONS:
1.Complete and maintain required training, credentialing, licenses, and background checks
2.Regularly report to work on days and times of approved schedule in order to accommodate client and business needs
3.Participate in centralized scheduling process
4.Provide direct individualized clinical services 60% of hours worked that may include:
5.Complete all CHCS and contract specific required documents in timely manner and to standard
6.Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
7.Comply with and participate in Quality Assurance and Quality Improvement processes
8.Coordinate coverage for planned absences in advance with supervisor
9.Adhere to established CHCS Employee Handbook and policies and procedures
10.Other job duties as assigned

Compensation Package
Salary Range Min $33,903.00 - Max $50,855.00
