Responsibilities:
Develop, coordinate, and oversee individualized care plans for homecare clients
Conduct client assessments and regular check-ins to monitor health, safety, and well-being
Serve as the primary point of contact for clients, families, and caregivers
Supervise and support direct care staff to ensure high-quality service delivery
Coordinate with healthcare providers, social workers, and community resources
Maintain accurate client records and documentation in compliance with regulations
Respond to client concerns, incidents, and changes in care needs in a timely manner
Requirements:
Experience in homecare, case management, or a related healthcare field
Strong knowledge of care planning, patient advocacy, and community resources
Excellent communication and interpersonal skills
Ability to manage multiple cases and prioritize effectively
Compassionate, patient-centered approach to care coordination
Proficiency with documentation and basic computer applications
Valid driver's license and reliable transportation preferred
