This position is remote, but the candidate needs to be in Nebraska.
Day to Day Responsibilities:
Conducting outbound calls to members to educate them on needed services and assist with overcoming barriers to care.
Scheduling appointments with provider offices or connecting members with appropriate resources when needed.
Accurately documenting all outreach attempts, member interactions, outcomes, and follow-up activities within the case management platform.
Maintaining confidentiality and compliance with HIPAA and organizational policies.
Collaborating with care management, provider, and quality teams to help improve member engagement and close gaps in care.
Must Have Requirements:
5+ years of social work experience.
Required Licensure/Certification:
Clinical Social Workers (ex. LCSW, LISCW) or Professional Counselors/Licensed Mental Health Counselors (ex. LPC/LPCC, LMHC) or Marriage and Family Therapists (LMFT) or Certified Alcohol and Drug Counselor (CADC) or Substance Use Disorder Professional (SUDP). Must be Certified to practice in Nebraska.
