JOB TITLE: Care Manager II - up to 19 hours/week

Salary Range: $90,000 - $120,000

Experience in senior care of religious organization preferred

JOB PURPOSE:
The Care Manager II provides comprehensive medical case management services to the Sisters of Mercy members. This position collaborates closely with individual Sisters to assess, plan, facilitate, and advocate for appropriate options and services to meet their medical and health needs. The Care Manager II also supports communication, education, identification of community resources, and coordination of services.

Rooted in the mission of the Sisters of Mercy, to respond with vision and compassion to the needs of those who are poor, sick, and uneducated, this role embodies compassion, mercy, and a commitment to social justice in all aspects of care and service.


Location:
This position serves Sisters of Mercy who reside in the local area and surrounding cities. Travel is expected to provide case

management services to sisters living in the community in Long Island- Nassau/Suffolk counties.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide medical case management services including assessment, care planning, coordination, and linkage with community providers.

  • Collaborate with team members to develop and implement individualized care and service plans.

  • Build and maintain professional, supportive relationships with Sisters and their caregivers.

  • Communicate regularly and effectively with community care and service providers to ensure a coordinated health care approach.

  • Educate and provide ongoing support to Sisters regarding their health and care options.

  • Accompany Sisters to medical appointments as needed and appropriate.

  • Communicate and coordinate with the Sister’s Life Minister and/or designated Power of Attorney for Healthcare.

  • Assist with enrollment and coordination of government healthcare benefits, medical equipment, and related services.

  • Provide clinical oversight and consultation for Sisters residing in Assisted Living, Rehabilitation, or Long-Term Care facilities.

  • Maintain accurate and confidential documentation of clinical contacts, assessments, and services provided.

  • Maintain privacy, confidentiality, and protect sensitive data.

  • Participate in departmental meetings, trainings, and continuing education.

  • May provide direction, guidance, or support to staff, contractors, or volunteers as needed.


QUALIFICATIONS

Required:

  • Registered Nurse (RN), Masters of Social Work (MSW), with at least five (5) years of relevant experience.

  • Minimum of two (2) years' experience working with older adults.

  • Experience in medical case management or care coordination.

  • Ability and willingness to work flexible hours and travel as required (by car and plane).

  • Valid driver’s license with a safe driving record.

  • Basic computer skills. - Teams, Outlook, Excel, Blackbaud, Concur

  • Knowledge and ability to self-manage time and work.

  • Excellent communication skills, both oral and written.

Preferred Qualifications:

  • Experience in long-term care, rehabilitation, or home health environments.

  • Familiarity with community-based resources, healthcare systems, and benefits navigation.

  • Understanding and appreciation of the mission and culture of the Sisters of Mercy and the Catholic Church.

  • Ability to work independently while maintaining strong collaboration within a team.

  • Strong organizational skills and ability to establish priorities.

  • Capable of displaying strong self-motivation and sound decision-making ability.
