Job Description:
- The Transitions of Care Coordinator will fulfill the critical Medicare MOC requirements related to sharing with admitting facilities a member’s plan of care as well as informing the PCP of record of their patient’s admission and discharge from the acute care setting, skilled nursing facilities and home care.
- Ensure current plans of care are sent to acute care facilities for admitted members.
- Ensure PCPs are notified of admissions to acute care facilities and discharge dates.
