Position Overview: 

We are seeking a compassionate, organized, and operationally focused Care Manager to coordinate the successful onboarding and ongoing support of AHCCCS members and their caregivers throughout Arizona. Working in partnership with ALTCS health plans, DDD Support Coordinators, physicians, and family caregivers, the Care Manager ensures members receive high-quality Home and Community-Based Services (HCBS) in accordance with authorized service plans while maintaining compliance with AHCCCS, EVV, and Entyre quality standards.

This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre’s internal teams to ensure a seamless and compliant care experience.

Job requirements

Key Responsibilities: 

Home visits: 

  • Conduct initial and ongoing home visits to evaluate member needs, develop Entyre service plans based on authorized services, assess caregiver competency, and identify opportunities to improve quality, safety, and member satisfaction.

Training & Support: 

  • Onboard new caregivers.

  • Complete competency evaluations.

  • Conduct supervisory visits.

  • Provide coaching.

  • Address performance concerns.

  • Support caregiver retention.

Reporting & Coordination: 

  • Coordinate care with AHCCCS health plans, ALTCS Case Managers, DDD Support Coordinators, physicians, hospitals, and community partners to support timely service initiation, care transitions, and ongoing member success.

Authorization Management

  • Review service authorizations issued by AHCCCS managed care organizations.

  • Ensure services delivered align with authorized service plans.

  • Monitor authorization expiration dates.

  • Coordinate renewal documentation with health plans.

Electronic Visit Verification

  • Ensure caregivers successfully utilize Electronic Visit Verification (EVV).

  • Monitor EVV compliance.

  • Resolve visit exceptions.

  • Support accurate documentation for billing.

Ongoing care: 

  • Facilitate smooth transitions and ongoing care for clients. 

Family Caregiver Success

  • Educate caregivers regarding program requirements.

  • Support caregiver confidence and long-term success.

  • Identify barriers to successful caregiving.

  • Connect caregivers with appropriate resources.

  • Promote caregiver engagement and retention.

Compliance: 

  • AHCCCS requirements

  • ALTCS requirements

  • DDD requirements (where applicable)

  • EVV regulations

  • Agency policies

  • Documentation standards

  • Incident reporting

  • Member rights

Community awareness: 

  • Build relationships with case managers, hospitals, physicians, community organizations, and referral partners to support member access to services.

Role Model:

Quality Assurance

  • Monitor member satisfaction.

  • Conduct supervisory visits.

  • Investigate incidents.

  • Implement corrective actions.

Documentation

  • Maintain timely documentation.

  • Complete assessments.

  • Maintain care plans.

  • Track authorizations.

  • Document member contacts.

 
What You Will Bring: 

  • Bachelor’s degree appreciated not required; preferably in social work, human services, or a related field. 

  • Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required. 

  • At least one (1) year of experience in-home healthcare.

  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting. 

  • Experience supervising caregivers preferred 

  • Preferred Experience with: AHCCCS, ALTCS, DDD, Medicaid HCBS, EVV & Home Care Home Base, Sandata, WellSky, or similar
