Job Title: Care Manager RN
Duration: 6-month contract |Possibility to extend or convert
Location: Hybrid (In-person & Remote Ohio) |Must reside in Northwest Ohio: Trumball, Stark, Seneca, Portage, Mahoning counties.)
Schedule & Training
- Training: Monday Friday, 8:00 AM 5:00 PM EST (5 weeks, 1on1-virtual, camera on, no time off allowed)
- Working Schedule: Monday Friday, 8:00 AM 5:00 PM EST
- Field Work: 50 60% | 5 7 visits weekly
- Remaining time: Remote documentation
- Occasional travel to Columbus, OH (once per year)
Pay rate: $44 to $49/hr
Responsibilities
- Develop, assess, and facilitate complex care management activities for primarily physical needs members
- Provide high-quality, cost-effective healthcare outcomes
- Create personalized care plans and provide education for members and their families
- Review emails, tasks, and voicemails and respond to urgent needs
- Review calendar for meetings and scheduled visits
- Conduct 5 7 member visits per week
- Complete documentation within 24 hours
- Meet turnaround times for required processes
- Complete required trainings

Requirements
Education
- Degree from an accredited School of Nursing with 2 4 years of related experience
Experience
- 2 4 years of experience in nursing, case management, home health experience
- Reliable vehicle required for weekly travel
