    Do you have what it takes to be a Care Manager (RN):

    •  Current Registered Nursing Licensure in the state of Ohio BA in Nursing preferred/ Associates required 2 to 3 years clinical practice experience Well-rounded clinical experiences including work in outpatient settings especially Family PracticeExperience with data analysis and health care operations 
     

    A day in the life of a Registered Nurse (Care Manager):

    • Assists patients through the healthcare system by acting as a patient advocate and navigator. Develops and implements Care Plans through Chronic Care Management Services Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visitsEducates patient and family on ways to reduce ER visits in the future Participates in Patient-Centered Medical Home team meetings, quality improvement initiatives and Value Based CareEvaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
