Summary: Work Mode: Remote & in-person visits in North East OH Location: Medina, Cuyahoga, Lake, Lorain, Geauga, Portage, Wayne, Summit counties Shift: Monday-Friday 8a-5p EST Responsibilities: Develop, assess, and facilitate complex care management activities for members with primarily physical needs. Provide high-quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. Conduct 5-7 in-home visits weekly, ensuring documentation is completed within 24 hours. Meet required turnaround times for processes and complete required training timely. Collaborate with coworkers and leadership to ensure members receive necessary care and services to remain independent. Requirements: Degree from an Accredited School of Nursing. Ohio RN Licensure required. 2-4 years of experience in nursing, case management, or home health. Reliable vehicle for weekly travel. Preferred Skills: 2-4 years of related experience in case management or home health. Computer literacy is a must, including knowledge of Microsoft Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint, Explorer, and Chrome. Strong critical thinking and independent working abilities. Personable with strong communication skills. Flexible and open to change, with the ability to work well in a team.
