Summary: Work Mode: Remote & in-person visits Location: North East OH (Medina, Cuyahoga, Lake, Lorain, Geauga, Portage, Wayne, Summit counties) OR North West OH (Lucas, Fulton, Wood, Ottawa counties) Shift: Monday-Friday 8a-5p EST Responsibilities: Develop, assess, and facilitate complex care management activities for physical needs members. Provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. Perform 5-7 in-home visits weekly, ensuring documentation is completed within 24 hours. Meet required turnaround times for processes and complete required trainings timely. Requirements: Degree from an Accredited School of Nursing. Ohio RN Licensure. 2-4 years of experience in nursing, case management, or home health. Reliable vehicle for weekly travel. Ability to work independently and manage change well. Proficient in Microsoft Office (Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint, Explorer, Chrome). Preferred Skills: Experience in case management or home health. Strong communication skills and ability to work well on a team. Critical thinker who is flexible and open to change.
