    Care Manager RN, Providence St. Lukes Rehabilitation Inst, Spokane WA

    $15,000 Sign-on bonus for eligible external candidates who meet all conditions for payment - this is an addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment.

    The External Case Manager performs activities and duties related to preadmission evaluation and admission of patients to services along the rehabilitation continuum of care. Also responsible for relationship building for marketing and patient census building and coordination of care in collaboration with the acute care Case Managers/Social Workers/Discharge Planners to facilitate a smooth transition and most appropriate level of care. The External Case Manager works with appropriate funding sources to establish the most effective manner of managing the human, material and financial resources available for the patient''s care.

    Required qualifications:

    • Upon hire: Washington Registered Nurse License
    • 3 years of Clinical experience.

    Preferred qualifications:

    • Bachelor''s degree in nursing.
    • Upon hire: CRRN and/or CCM certifications
    • Experience in case management; clinical nursing or utilization management.
    • Experience in the rehabilitation of chronically or catastrophically ill or injured individual.

    Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
