About the Role:
HomeWell Care Services CA305 is looking for a passionate and dedicated Care Manager to join our growing team in Upland, CA, serving San Bernardino County. Responsibilities:
- Conduct client assessments and develop individualized care plans tailored to each client's needs
- Coordinate and oversee in-home care services for clients throughout San Bernardino County
- Serve as the primary point of contact for clients, families, and caregivers to ensure quality care delivery
- Monitor and evaluate client progress, adjusting care plans as needs evolve
- Supervise caregiving staff to maintain high service standards
- Build and maintain relationships with referral sources, healthcare providers, and community partners
- Ensure compliance with state regulations, company policies, and care standards
Requirements:
- Experience in home care, case management, or a related healthcare field
- Strong knowledge of in-home care services and San Bernardino County resources
- Excellent communication, organizational, and interpersonal skills
- Ability to manage multiple clients and priorities in a fast-paced environment
- Compassionate, client-centered approach to care coordination
- Valid California driver's license and reliable transportation for county-wide travel
- Experience with caregiver supervision and staff management preferred
