Alegre Home Care is seeking a compassionate, organized, and proactive Care Manager to join our team. The Care Manager plays a key role in ensuring clients receive exceptional care and customer service while serving as a liaison between clients, caregivers, and office staff.
This position combines client care management, caregiver support, community outreach, and business development. The ideal candidate is a strong communicator, enjoys building relationships, and is passionate about helping seniors and their families navigate home care services.
Key Responsibilities
Client Care Management
- Participate in handling and responding to client intake inquiries.
- Conduct initial client assessments and determine care needs.
- Develop individualized care plans in accordance with Alegre Home Care policies and procedures.
- Enter, update, and maintain care plans within the client management system.
- Monitor client satisfaction and ensure care plans remain effective and up to date.
- Communicate regularly with clients, family members, and designated contacts.
- Provide exceptional customer service and promptly address client concerns.
Caregiver Support & Supervision
- Review care plans and client needs with caregivers as necessary.
- Assist in supervising and supporting home care aides.
- Collaborate with scheduling staff to ensure appropriate caregiver-client matches.
- Conduct routine supervisory visits to monitor quality of care and caregiver performance.
- Assist with problem-solving and resolution of client or caregiver concerns.
Community Outreach & Business Development
- Attend networking events, community meetings, and industry functions.
- Build and maintain relationships with healthcare professionals, senior living communities, and referral sources.
- Support business development efforts to increase client referrals and community awareness.
- Oversee the Alegre Quick Care Program at Villa of San Mateo.

Preferred Qualifications
- Experience in home care, healthcare, senior services, or a related field.
- Supervisory or management experience.
- Knowledge of client care planning and care coordination.
- Associate or Bachelor degree preferred.
Requirements
- High school diploma or equivalent required.
- Valid driver’s license and reliable transportation.
- Ability to travel locally for client assessments, supervisory visits, networking events, and community meetings.
- Ability to maintain confidentiality and professionalism when working with clients and families.
