CARE MANAGER JOB DESCRIPTION

What You’ll Do

As a Care Manager, you will provide in-home support and companionship to individuals in our community while helping them live safely and comfortably in their own homes. Responsibilities may include:

• Assisting clients with Activities of Daily Living (ADLs), including:
  • Bathing
  • Dressing
  • Grooming
  • Toileting
  • Mobility assistance
  • Meal preparation
• Medication reminders
• Light housekeeping and laundry
• Providing companionship and emotional support
• Assisting with errands and community activities
• Maintaining accurate documentation and communication with the care team
• Creating meaningful connections with clients and families through compassionate care

Travel Requirements

This position requires travel between client homes throughout the shift. Care Managers must have:

• A valid driver’s license
• Reliable transportation
• Current auto insurance
• The ability to safely travel to multiple client locations as assigned
