The Area Office on Aging of Northwestern Ohio (AOoA) is dedicated to helping older adults and individuals with disabilities remain healthy, independent, and living in their homes and communities. Serving over 30,000 individuals annually, our team is committed to delivering compassionate, person-centered care across our region. Position Overview Care Manager (also known as a Waiver Service Coordinator) is responsible for conducting in-home assessments, identifying support systems, and coordinating long-term services through the MyCare Ohio Waiver Program. This role ensures members receive appropriate care in home- and community-based settings while maintaining compliance with Medicaid and program regulations. What You’ll Do
