Care Navigator

Location: Sacramento, CA 95825

Duration: 4-6+ months contract

Introduction

The Care Navigator plays a crucial role in ensuring that patients receive the appropriate care and support they need. This position requires a compassionate individual who can effectively communicate and coordinate with various healthcare professionals and patients.

Required Skills & Qualifications

• High school diploma with 2-5 years of relevant experience required; bachelor’s degree in social work, Psychology, Public Health, Human Services, Sociology, Gerontology, or a related field, preferred.
• Bilingual and bicultural skills reflective of the communities served.
• Experience in an Emergency Department (ED) or Inpatient setting is highly preferred/required.
• Comfortable in a clinical setting, interacting with Nurses, Social Workers, and independently introducing themselves to patients and families.
• Applicants must be able to work directly for Artech on W2.

Preferred Skills & Qualifications

• Prior work experience at the client or in the client's industry.
• Familiarity with healthcare systems and patient care coordination.
• Strong organizational and multitasking skills.

Day-to-Day Responsibilities

• Coordinate care for patients by liaising with healthcare providers and ensuring seamless communication.
• Provide support and resources to patients and their families, helping them navigate the healthcare system.
• Assess patient needs and develop care plans in collaboration with healthcare professionals.
