The Care Partner is a compassionate, person-centered caregiver dedicated to supporting our Kupuna through engaging activities, quality care, and daily living support. Care Partners foster a respectful, culturally sensitive environment that values participant preferences while promoting health, safety, and enrichment. Working closely with the Program Manager, Care Team, and Nursing staff, the Care Partner provides both social engagement opportunities (recreation, arts and crafts, music, exercise, outings, and educational enrichment) and hands-on assistance with personal care.
This position is ideal for individuals who are passionate about making a difference in the lives of Kupuna and are committed to delivering exceptional, individualized care.
Responsibilities and Duties:
Direct Participant Care
- Provide respectful, age-appropriate, and culturally sensitive interactions with participants, honoring individual preferences and care plans.
- Support participants with ADLs (Activities of Daily Living), including toileting, bathing, ambulating, transfers, feeding, changing clothes, and other personal care needs.
- Monitor participants’ health and report illness, incidents, or changes in condition to the Nurse/Floor Supervisor.
- Ensure safety of all participants by monitoring activities, maintaining a clean, sanitary, and clutter-free environment, and participating in safety training and emergency drills.
- Immediately report suspected abuse, neglect, or exploitation per program and agency policies.
- Assist with serving meals and snacks according to USDA guidelines and provide feeding assistance as needed.
- Employ assistive equipment and safe practices when transporting, lifting, or supporting
participants. - Keep anecdotal notes and prepare monthly progress reports on assigned participants.
- Maintain confidentiality while handling participant health and personal information.
Activity and Program Support
- Lead or assist with stimulating activities such as crafts, table games, music, exercise, and
educational enrichment. - Participate in planning and supporting community outings and the monthly activity
calendar. - Encourage participant engagement, inclusion, and enjoyment during all programs.
CNA Specific Duties (if certified)
- Administer medications and assist with wound care under Program Manager/Nurse
supervision. - Knowledge and competency to access, interpret and care for participants in the age
group of 60+. - Cleans up spills, bodily fluids, etc. using appropriate infection control procedures.
- Perform showers and personal hygiene support as scheduled or requested.
- Provide additional skilled CNA care as appropriate to participant needs.
Program Operations
- Maintain program areas, supplies, and equipment in a clean, disinfected, stocked, and
safe condition. - Document incidents, follow emergency procedures, and stay updated on all safety
protocols. - Support accurate and timely documentation in participant electronic health records.
- Use basic computer skills for email, recordkeeping, and communication.
- Participate in professional development, training, and team meetings.
- Demonstrate flexibility in adapting to changes in routines, tasks, or schedules.
- Perform other related duties as assigned by the Program Manager or leadership.
Requirements and Qualifications:
Required:
- High school diploma or GED.
- CPR/First Aid certification (training available upon hire).
- Two-step TB clearance.
- Recent physical examination.
- Strong interpersonal, organizational, and communication skills.
- Ability to work both independently and as part of a team.
- Commitment to confidentiality, professionalism, and cultural competency.
- Basic computer skills and ability to learn participant database systems.
Preferred / CNA Track:
- Verified and current Certified Nursing Assistant (CNA) certification.
- Experience working with elderly or disabled adults.
- Valid Hawai?i Driver’s License or State ID.
- Interest in memory care and life enrichment activities.
- Basic technical proficiency and communication skills to use and support participant
electronic health records in our participant database. - Working knowledge of standard Microsoft Office software applications including email.
Physical Requirements:
- Ability to life and carry up to 25 lbs as needed.
- Ability to stand and/or engage with participants for 6–8 hours per day.
- Must be able to operate general office equipment and work at a desk
- Assist participants with walkers, wheelchairs, and transfers (two-person assist when
necessary). - Physically able to support participants during fire drills, outings, or emergency
situations. - Move tables, chairs, and program supplies as needed.
- Must be able to perform all duties of the position, with or without reasonable
accommodation, in accordance with the law.

Expected Hours of Work:
Part or full-time position to include a Saturday rotation and most holidays (30+hours/week). All HIAC employees have Sunday off.
