Care Services Representative

Position Details

We are seeking a full-time Care Services Representative to join our Care Coordination Center in Centerville, Ohio. This onsite position is scheduled for 40 hours per week, with shifts between 8:00 a.m. and 6:30 p.m. Team members participate in an every-third-weekend rotation and work 1-2 holidays each year on a rotating basis.

This role supports patients, families, referral sources, and care teams across Ohio's Hospice service areas. If you're compassionate, service-oriented, and thrive in a fast-paced environment, we'd love to hear from you.

Position Overview

As a Care Services Representative, you'll be one of the first points of contact for patients, families, healthcare providers, and community partners. You'll provide exceptional customer support, gather accurate information, coordinate communication, and help ensure every interaction reflects our commitment to compassionate hospice care.

Primary Responsibilities

  • Respond to incoming calls and inquiries with professionalism, empathy, and outstanding customer service.
  • Collect and document accurate patient and referral information.
  • Clearly explain available hospice services and answer questions.
  • Maintain complete and timely documentation of all interactions.
  • Collaborate with teammates and assist other departments as needed.
  • Participate in quality improvement initiatives that enhance patient and family care.
  • Support the onboarding and training of new team members when assigned.

Qualifications

  • High school diploma or GED required.
  • Strong computer and data entry skills.
  • Excellent communication and customer service abilities.
  • Valid driver's license with a safe driving record.
  • Proof of state-required automobile insurance.
  • Ability to travel in various weather conditions when necessary.
  • Two years of customer service experience preferred.
  • Ability to successfully complete a criminal background check and 10-panel drug screening.
