Job Summary and Responsibilities
At Post Health Home Care and Hospice we strive to embody our mission of delivering hope and healing to those we serve. As a Care Transition Liaison (CTL-LPN) withPost Health Home Care and Hospice, you will play a key role in providing coordinated home health services for referred patients. You'll collaborate with healthcare professionals, identify patient needs, and facilitate seamless transitions to home health care.
Job Requirements
Graduate of an accredited school of nursing, current LPN license to practice in the state serviced
Minimum of (2) years of healthcare experience, preferably in home health or sales/marketing.
Strong communication, assessment, and decision-making skills.
Ability to build relationships, educate, and promote services effectively.
Committed to professional development and improving own skills.
