GENERAL SUMMARY:
The Care Transitions Manager provides leadership and supervision of the key functions, roles, systems and processes related to care transitions and population management into and out of the medical center. Assures the delivery of quality, patient centered care in a manner reflective of the organizations' mission, vision, values and beliefs. Possess managerial and clinical skills appropriate to the scope of responsibility. Manages the cost centers' fiscal, material and human resources emphasizing staff retention strategies. Participates in department, division and organizational wide initiatives focusing on regulatory compliance and performance improvement.
PRINCIPAL JOB FUNCTIONS:
EDUCATION AND EXPERIENCE:
Master's degree in Social Work required. Licensure as a Licensed Clinical Social Worker (LCSW) and Licensed Mental Health Practitioner (LMHP) by the State of Nebraska required. Minimum of three (3) years recent clinical experience required. Prior care transitions management experience preferred. Prior supervisory or management experience preferred. Case Management certification preferred.
OR
Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Bachelor's degree required, master's degree preferred. Prior care transitions management experience preferred. Prior supervisory or management experience preferred. Case Management certification preferred.
OTHER CREDENTIALS / CERTIFICATIONS:
Basic Life Support (CPR) certification required.
