Career Coach

 

Under the designated supervisor, the Career Coach’s job function is to help youth in One Hope United Transitional Living Programs develop workplace readiness skills, find and obtain internships, and to grow as young professionals in a work-based learning environment.

Essential Duties and Responsibilities 

• Responsible for the development of individualized transition plans written in collaboration with youth in care. Plans should include a vision and goals for youth after graduation from TLP in the following areas: Employment, Community Living, Healthy Living, Social & Spiritual Life, Safety & Security, and Citizenship. 
• Work will be performed at the home of participants, in mutually agreed upon community settings, and at assigned work sites in the community. The Career Coach is responsible for the safety and well-being of the assigned service recipients and is required to comply with all agency standards, policies and procedures, assuring quality services are provided on a daily basis.
• Be a productive team member; promote a positive work environment regarding service recipients, co-workers, family members, customers, employers and community stakeholders with courtesy and respect.
• Responsible for supporting youth in reaching self-identified goals and outcomes according to the individualized transition plan. Must keep abreast of changing needs of service recipients and adapt to new research as to best practices in supported employment and work with youth in care.
• Responsible for the supervision of assigned service recipients at specified times to assure their safety and well-being and provide support and guidance in needed areas. The level of supervision and pace of service fading should be individualized to each participant.

• Implement program curriculum and adapt where necessary to promote independence for service recipients, develop workplace readiness skills, and meet client goals.
• Monitor service recipients for behavioral or medical issues and report in a timely manner.
• Complete incident reports and other necessary documentation in a timely manner. 
• Perform the duties of Career Coach assisting service recipients in the development of workplace supports to assure the best possible opportunity for successful competitive employment.
• Provide on-going training and support to service recipients in work ethic, conduct, and performance. 

• Support service recipients in obtaining and maintaining meaningful and relevant internship opportunities.
• May be responsible for supporting youth in requesting and implementing workplace accommodations, using a strong understanding of Title I of the Americans with Disabilities Act and Section 504 of the Rehabilitation Act of 1973.
• May be responsible for transporting service recipients to and from employment sites as assigned. Alternatively, may be responsible for providing training in use of public transit or supporting youth in setting goals that support independence in transportation.
• Adhere to all federal, state, and local laws regarding the safe operation of motor vehicles, including the use of adaptive devices/ equipment on vehicles. 
• Participate in staff team meetings and attend required staff training. 
• Comply with One Hope United Policy and Procedures and Employee Handbook.
• Maintain confidentiality, HIPAA requirements, and Title VI.
• Maintain flexibility and commitment to service recipient success regarding schedule. Internship placements and employee hours should be arranged to the youth’s availability, rather than the Career Coach.
• Other duties as assigned.

• Maintain a positive work atmosphere by acting and communicating in a manner so that you get along with customers, people supported, co-workers, direct reports, and management. 
• Ensure Person Centered and strength-based practices, a trauma-informed approach, and respect for autonomy of participating youth.
• Promote the professional and personal development of the youth. Model expected behaviors and serve in a mentoring role.
• Ensure all written reports, letters and any other form of communication represents OHU in a professional manner.

Education: High school diploma/GED. Bachelors in related field preferred.

 

Experience: Minimum two years’ experience in residential care, career coaching or 
related field or a social service-related field. Five years preferred.
 

Other: Valid driver’s license and acceptable driving record with OHU required 
levels of coverage for auto insurance

 

• Must possess effective oral and written communication abilities. 
• Must maintain current CPR/First Aid certification and meet all DCFS and agency training requirements.
• Possess the ability to handle multiple tasks.
• Understanding of how trauma impacts behavior.
• Highly organized, yet flexible and can handle multiple projects and or priorities well with the ability to manage tight deadlines.
• Intermediate data entry skills 
• Resourcefulness in researching issues, analyzing information skillfully, solving problems, and developing solutions with minimal supervision in a timely manner.
• Proficient computer skills in a Microsoft Windows environment and adaptive skills to expand software application skills as necessary.
• Ability to exercise discretion and ensure confidentiality.
