Job Descriptions
PCA / CFSS (Personal Care Assistance / Community First Services and Supports)
Caregiver providing these services support individuals in their home and communities with authorized Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as outlined in the clients service plan.
Duties may include assistance with personal cares such as bathing, dressing, eating, mobility and toileting, as well as support with household tasks, medication reminders (not administration), range of motion exercises when authorized, and accompanying clients to appointments for support. All services are performed according to the approved care plan, and caregivers may not perform duties outside of what is authorized.
Caregivers are responsible for accurately documenting services provided and reporting changes in the client's condition as required.
245D Services
Caregivers providing services under Minnesota's 245D Home and Community-Based Services support individuals in maintaining independence, safety and quality of life in their homes and communities. Services are delivered according to each client's service plan and within the scope of the authorized service.
245D services may include routine household support, assistance with daily living skills and routines, community participation, temporary relief care for primary caregivers, and overnight monitoring to ensure safety. Services offered by All Home Health Inc. include Homemaker, Individualized Home Supports (IHS), Independent Community Living Supports (ICLS), Respite Care, and Night Supervision, with supports tailored to the individuals assessed needs.
Training & Onboarding Requirements
All Caregiver must complete DHS-approved training and meet agency training and onboarding requirements before providing client serviceds. This includes completion of required online training, in-service and hands-on training as applicable, and submission of required certificates or documentation. Caregivers providing PCA/CFSS services must also complete DHS enrollment and certification requirements prior to working independently.

Basic Skills & Abilities:
Mandated Reporter:
All positions are Mandated Reporter roles under Minnesota law. Employees are legally required to report suspected abuse, neglect, or financial exploitation of a vulnerable person when there is reason to believe it has occurred.

While performing job duties, the employee may be required to use hands, stand, walk, reach, bend, kneel, or crouch. The employee may occasionally be required to sit and to lift or move up to 50 pounds.
Background Check Requirements:
All applicants are subject to criminal background studies, conducted through the Minnesota Department of Human Services (DHS) in accordance with Minnesota Statutes, Chapter 245C. Individuals with disqualifying offenses or substantiated maltreatment findings may be ineligible for employment unless rehabilitation criteria are met. Employment will not begin until the background study is completed and eligibility is confirmed.
