Position Overview

Ceresti Health is seeking a compassionate and motivated Dementia Caregiver Coach to provide remote support, coaching, and education to family caregivers of loved ones living with dementia. This vital role is at the heart of our innovative Caregiver Support Program, a first-of-its-kind digital health model designed to improve health outcomes, reduce avoidable hospitalizations, and enhance the experience of caregiving for families impacted by Alzheimer's disease and related dementias.

The ideal candidate will have experience working with individuals living with dementia, be confident managing a caseload, and excel at building relationships through empathy, encouragement, and education. Key Responsibilities

  • Deliver high-quality, person-centered coaching to family caregivers of individuals living with dementia, tailored to their unique challenges.
  • Use Ceresti's proprietary technology platform to manage daily workflow, track caregiver progress, and document caregiver interactions.
  • Initiate and maintain consistent outbound engagement to establish trust and build strong, long-term relationships with assigned caregivers.
  • Educate caregivers on the dementia disease process, common behavioral symptoms, safety strategies, and techniques for managing stress and preventing burnout.
  • Coach caregivers through emotionally complex scenarios, such as end-of-life conversations, transitions to facility care, or family conflict around care decisions.
  • Guide caregivers in care planning, appointment preparation, accessing community-based resources and care coordination, helping them communicate effectively with healthcare providers and navigate the healthcare system.
  • Meet or exceed performance metrics related to caregiver engagement, satisfaction, and program outcomes.
  • Attend and contribute to regular team huddles, trainings, and case reviews, sharing insights and best practices with peers and leadership.
  • Participate in quality improvement initiatives, including pilot studies, internal feedback loops, and caregiver experience evaluations.
  • Other duties as assigned.

Required Qualifications

  • 2+ years of experience working professionally with older adults or persons living with dementia.
  • Direct caregiving experience (personal or professional) with persons with cognitive impairment.
  • Fluency in English.
  • Must be eligible to work in the U.S.
  • Ability to work independently in a fully remote setting.

Preferred Qualifications

  • Bachelor's degree in psychology, social work, human services, or a related field.
  • Direct caregiving experience (personal or professional) with persons with cognitive impairment.
  • Experience with caregiver support, family systems, or dementia-related behavioral challenges.
  • Excellent communication, problem-solving, and active listening skills.
  • Comfort navigating technology and learning new platforms (Mac/IOS).
  • Strong organizational and time management abilities.
  • Previous experience in a HIPAA-compliant setting.
  • Empathetic, resourceful, and confident managing emotionally complex situations.
  • Ability to thrive in a fast-paced, change-driven environment.
