Caregiver Support Coordinator (Part-Time – 25 Hours/Week)

What You'll Do

As JABA's Caregiver Support Coordinator, you'll serve as a trusted guide for caregivers and individuals seeking information, support, and long-term planning.

In this role, you will:

  • Provide person-centered information, assistance, and options counseling to caregivers and the individuals they support.
  • Conduct caregiver assessments and help develop individualized care plans based on each family's goals, strengths, and preferences.
  • Connect caregivers with community resources, public benefits, respite services, and other supports.
  • Facilitate caregiver support groups that foster education, encouragement, and peer connection.
  • Develop caregiver education materials, resource guides, and helpful online tools.
  • Maintain current knowledge of local, regional, and statewide services available to older adults and caregivers.
  • Educate community members, professionals, and partner organizations about JABA programs and caregiver resources.
  • Maintain accurate and timely documentation in agency databases and prepare required program reports.
  • Participate in outreach events, educational presentations, and community collaborations that strengthen caregiver support throughout our region.

What We're Looking For

We're seeking someone who combines professional expertise with genuine compassion.

Our ideal candidate has:

  • A Bachelor's degree in Human Services, Social Work, Gerontology, Public Administration, Psychology, or a related field.
  • At least three years of experience in human services, aging services, care coordination, case management, caregiver support, or a related field.
  • Knowledge of community resources, long-term services and supports, and public benefit programs.
  • Excellent communication, listening, and relationship-building skills.
  • Strong organizational skills with the ability to manage multiple priorities while maintaining accurate documentation.
  • Experience facilitating groups, conducting presentations, or providing community education is highly desirable.
  • A commitment to person-centered service, cultural humility, and treating every individual with dignity and respect.

Schedule

This is a part-time position (25 hours per week), typically working Monday through Friday, 9:00 AM – 2:00 PM. Occasional flexibility is required to facilitate caregiver support groups, attend community events, or participate in meetings outside normal business hours.

Travel

Local travel throughout the JABA service area is required to conduct home visits, attend meetings, facilitate support groups, and participate in community outreach.
