\nOur Wellness Partners provide one-on-one care guided by individualized, nurse-led Wellness Plans. Care may include personal care, mobility and transfers, dementia support, meals, medication reminders, companionship, and purposeful daily activity.
\nBut how the care is delivered is what sets this role apart.
\nWe’re looking for caregivers who notice the little things. Who take initiative. Who protect dignity. Who communicate when something seems off. And who genuinely want to help seniors maintain their strength, independence, confidence, and quality of life.
\nQualifications
\n* 1+ year professional caregiving experience
\n* Hands-on personal care/ADL experience
\n* Strong communication, observation, and judgment
\n* Valid driver’s license, reliable transportation, and auto insurance
\n* Willingness to travel throughout Toledo and surrounding Northwest Ohio
\n* Ability to complete required background, TB, and drug screening
\nPreferred: STNA/CNA certification, dementia-care experience, and experience with mobility, transfers, and fall prevention.
