Transitions At Home is seeking a compassionate, dependable PRN Caregiver to provide exceptional in-home care for clients in the Lyndon Station area.

Primary Responsibilities:
- Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene
- Prepare meals and assist with cooking
- Perform light housekeeping and laundry
- Provide safe transfers and mobility assistance
- Offer companionship and meaningful social engagement
- Brighten each client's day with friendly, compassionate support
Preferred Qualifications:
- Experience with catheter and ostomy care is a plus
- Willingness to learn—we are happy to train the right candidate
- Compassionate, reliable, and patient demeanor
- Ability to work independently while providing high-quality care
- Strong communication and interpersonal skills

Hours: 10 Hours a Week
- Monday 10am-1pm
- Friday 10am-4pm
- No Holidays
- No Weekends

Qualifications / Job Requirements:
- Must be a minimum of 18 years of age
- One year of Caregiving experience preferred
- A valid Wisconsin Driver's License and Car Insurance
