The Case Assistant reports to the Ryan White Clinical Quality Manager and provides support services for Hidden Garden Medical Case Management as instructed.
Responsibilities:
Monitor clients' compliance with primary medical care, mental health, and substance use appointments.
Provide pre-appointment reminders; assess barriers to keeping appointments and assist with rescheduling HIV primary care, behavioral health and specialty appointments.
Monitor eligibility for Ryan White services and update agency forms.
Make phone calls to and on behalf of clients and conduct home visits for lost to care clients.
Arrange transportation and/or accompany patients to appointments as requested
Qualifications:
Bachelor's Degree with two years' experience working in a social service environment
Relevant background experience and sensitivity to HIV/AIDS related issues and African-American and other minority community issues.
An awareness of local AIDS services and resource providers at the public and community levels.
