Our housing‑first strategy ensures that every Veteran family we serve secures permanent housing. We support Veterans through federal Veterans Affairs and state funding opportunities, such as Supportive Services for Veteran Families (SSVF).ResponsibilitiesCollaborate with Veterans and other team members to identify and secure affordable housing. Provide direct services to Veterans and their families who are homeless or at imminent risk, addressing mental health, physical disabilities, substance abuse, and other challenges. Maintain weekly communications with each Veteran on the caseload.Develop Housing Stability Plans (HSP) and action steps to achieve and maintain permanent housing. Work with Veterans to increase income for housing stability. Maintain detailed case notes and enter them weekly in HMIS (Clarity). Manage all paperwork and meet coinciding deadlines.Maintain positive, professional communications and relationships with Veterans, household members, and collateral contacts. Provide ongoing referrals and resources, documenting on the Referral Log and case notes. Complete all required training within deadlines.Comply with all safety standards and practices. Transport Veterans to appointments, housing searches, and other necessary locations, ensuring safety and timeliness.RequirementsEducation & Experience: Bachelor's degree or two years of equivalent experience in working with veterans and/or individuals experiencing homelessness. Must be 18 years of age or older, and successful completion of a pre‑employment drug screen and background check.
