Performs systematic ongoing assessment of Nurse Case• Manager and Social Work needs with focus on transitioning• patients safely and expeditiously out of the inpatient setting• 2. Audits assigned processes and aggregates the data for• leadership review• 3. Assists with DME inventory management• 4. Updates white board as needed• 5. Assists with Homeless supply management• 6. Maintains daily discharge and transportation logs• 7. Assists with employee timekeeping and scheduling• 8. Takes minutes during Department Staff Meetings• 9. Manages incoming and outgoing department mail• 10. Faxes referrals to post-acute providers and follows up as• necessary• 11. Manages department SharePoint resource space• 1.
