Case Management Assistant, New Inpatient Rehabilitation Hospital (2969)

Reporting to the Manager of Case Management, the Case Manager Assistant supports the case management team in coordinating a smooth and timely discharge process. This role facilitates discharge services, appointments, DME needs, and access to community resources. The ideal candidate is organized, communicative, compassionate, and able to problem-solve effectively.

Job Responsibilities

• Coordinate post-discharge services, appointments, and required documentation.
• Collaborate with DME providers to ensure equipment is available at discharge.
• Communicate effectively with patients and families regarding discharge needs.
• Assist with connecting patients to community resources and support services.
• Support team conference documentation and compliance requirements.
• Maintain accurate and timely records.
• Promote a positive patient experience through compassionate communication and service.

Job Qualifications

• High school diploma or equivalent required.
• One year of healthcare experience preferred.
• Strong verbal and written communication skills.
• Computer proficiency.
• Excellent organizational and time-management skills.
• Strong problem-solving and follow-through skills.
