Job Title: Case Management Assistant - Remote - MUST LIVE IN NV
Job Description
The Case Management Assistant plays a crucial role in supporting discharge planning alongside Case Managers and Social Workers. This position involves assessing post-hospital needs, coordinating referrals, authorizations, and transportation services, and communicating patient preferences and discharge timelines. The role requires collaboration with payers to provide benefit coverage information, monitoring progress toward discharge goals, facilitating transfers to post-acute facilities, and supporting utilization review transmission. Additionally, the assistant will manage high-volume faxes, voicemails, and data entry, participate in daily rounds, and maintain accurate documentation in Epic EHR and Allscripts.

Work Environment
This is a fully remote position, offering the opportunity to gain experience working for a large hospital system from home. The position requires the ability to work rotating weekends and adhere to a schedule of 10am-6:30pm PST, regardless of the candidate"s location. It offers the potential for a long-term contract, with consultants having previously been on contract for over a year.

Pay and Benefits
The pay range for this position is $18.00 - $18.00/hr.
