Requirements:
- High school diploma, GED, or equivalent education
- 1 years of recent relevant experience
- Experience with healthcare operations, care coordination, or related administrative workflows
- Ability to coordinate transition-of-care and discharge-related activities across multiple stakeholders
- Proficiency in intermediate PC, word processing, and electronic documentation
- Strong oral and written communication skills
- Ability to work independently with minimal direction
- Ability to prioritize competing tasks in a fast-paced environment
- Ability to exercise discretion and seek input when appropriate
Preferred Requirements:
- Experience with skilled nursing facility admissions or intake
- Experience working in a skilled nursing facility or post-acute care setting
- Experience with insurance-related workflows or payer coordination
- Experience communicating with healthcare providers, facilities, families, and external vendors
