How you'll make an impact in this role:

• Secure and document insurance authorizations for ancillary services, track denial reporting, and maintain continuous communication with payers to update internal case management staff on coverage status.
• Expedite discharge timelines by compiling and securely transmitting clinical documentation to external facilities; coordinate patient transfer logistics in close alignment with clinical teams.
• Facilitate, track, and document all patient referrals to post-acute care facilities, home health agencies, and community resources, ensuring compliance with patient choice standards.
• Serve as a central point of contact and communication hub connecting case management, admissions, and insurance representatives to resolve operational bottlenecks.
• Perform comprehensive service support for the case management department, ensuring high accuracy and audit-ready documentation across all interactions.

What minimum qualifications you'll need:

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
  • Note: Required professional licensure/certification can be used in lieu of education OR experience, if applicable.

What additional requirements you'll need:

• Prior administrative or entry-level case management support experience in a healthcare or social work setting.
