Case Management Lead - Supportive Housing

36-hour work week | Flexible hours | Salt Lake City, Utah

$60,000-$65,000 annually

We're looking for a Lead Case Manager to join our supportive housing team at The Point. In this role, you'll provide leadership, supervision, and support to case managers while helping ensure our residents receive high-quality, person-centered services.

You'll work closely with case managers, housing partners, and community providers across several supportive housing communities, including The Point Airport, Howick, Birkhill Lofts, Richmond Flats, and Fire Clay.

What You'll Do

• Lead, train, supervise, and support a team of case managers
• Provide guidance and mentorship to help staff succeed
• Coordinate services and collaborate with housing and community partners
• Help manage caseloads and ensure residents receive consistent support
• Monitor case management documentation and quality of services
• Support program development, implementation, and ongoing improvement
• Assist with grant reporting, budgets, and program requirements
• Provide direct case management coverage when needed
• Help resolve concerns between residents, case managers, and partner organizations
• Maintain accurate documentation and information in UHMIS and other required systems
• Ensure services and documentation follow applicable program, legal, and ethical requirements
• Participate in staff meetings, trainings, and other organizational activities

What We're Looking For

• Experience in case management preferred
• Experience working in housing or supportive housing preferred
• Bachelor's degree in social work or a related field preferred
• Experience working with individuals experiencing substance use or mental health challenges preferred
• Strong communication, organization, and leadership skills
• A genuine interest in supporting people as they work toward stability and recovery
