Care Management Coordinator (Remote - Texas)

Location: 100% Remote (Candidates must reside in Texas)
Pay Rate: Starting at $25.00 per hour
Duration: 1 year contract

Schedule

  • Monday through Friday
  • 8:00 AM - 5:00 PM CST
  • 1-hour lunch break

Assignment Duration

Contract assignment anticipated through September 15, 2027. No expectation of conversion to a full-time employee position.

Position Summary

The Care Management Coordinator provides non-clinical administrative support to the Care Management team and contributes to coordinated care delivery across the healthcare continuum. This role assists with member support activities, case management coordination, documentation, reporting, and communication with healthcare providers and interdisciplinary care teams. The ideal candidate will possess strong organizational skills, healthcare administrative experience, and the ability to work effectively in a fast-paced, remote environment.

Key Responsibilities

  • Provide administrative support to Care Management staff while assisting with departmental operations.
  • Perform initial review and triage of care management tasks.
  • Review data to identify member needs and support implementation of care plans under the direction of clinical staff.
  • Screen members using established policies, procedures, and workflows to support care management activities.
  • Coordinate services in accordance with member benefit plans and care requirements.
  • Promote effective communication between healthcare providers, care teams, members, and other stakeholders.
  • Generate and analyze reports to support care management activities and member needs.
  • Support team members by answering phone calls, taking messages, conducting research, and gathering information.
  • Maintain accurate, timely, and complete documentation in accordance with regulatory, accreditation, and risk management standards.
  • Ensure confidentiality of member information and maintain compliance with HIPAA and privacy regulations.
  • Contribute to interdisciplinary team efforts supporting quality, cost-effective, and integrated care delivery.

Required Qualifications

  • High School Diploma or GED.
  • 1-3 years of administrative experience in a healthcare environment.
  • Strong customer service and member support skills.
  • Experience working in a fast-paced environment with competing priorities.
  • Ability to work independently and collaboratively as part of a team.
  • Proficiency in Microsoft Windows-based computer systems.
  • Accurate data entry skills with a minimum typing speed of 40 WPM.
  • Knowledge of HIPAA regulations and patient confidentiality requirements.
