Position Overview:The Case Management Program Director oversees the agency's case management program and provides leadership to staff serving adults age 60 and older. This role supervises case managers who work with older adults and, when appropriate, their families to identify service needs, develop care plans, and help clients achieve mutually agreed-upon goals. The Program Director ensures program compliance with NYC Aging standards, supports quality service delivery, and represents the agency in the community. This position also oversees program operations, staff development, reporting, and collaboration across departments.
