Schedule/Hours: Monday through Friday, 8:00 am - 4:30 pm (40 hrs. per week)

This position is responsible for:

Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients in an established life management program. Develops program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff.

Key Responsibilities:

  • Interviews and conducts needs assessment to determine eligibility for program based on guidelines and requirements. Make referrals to other agencies if not appropriate for program.
  • Prepares and maintains case records of assigned clients. Enters pertinent information into the Homeless Management Information System (HMIS).
  • Establishes comprehensive goals for eligible clients May accompany and/or provide transportation for clients to meetings related to case plan.
  • Plans, coordinates and/or facilitates life management, behavior modification, etc. classes to assist clients.

Physical Requirements and Working Conditions:

  • Work is also performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.

  • Bachelor's degree or greater from an accredited college or university in Social Work, Behavioral Science, or a related field AND
  • Three years progressively responsible experience providing direct case management social services or
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Licenses and Certifications:

Valid Driver's License.
