As a Case Manager Supervisor, you will help to oversee our CCBHC Case management program located in Mt. Holly, Burlington County, NJ. You will be working with adults with mental illness and/or substance use disorders and children with serious emotional disturbance. Our programs service adults, children, and families. Your role will be essential to our success, and your efforts will influence our ability to provide treatment to our community.

COMPETENCIES: The potential employee must have the ability to assess and interpret information related to population; Knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; General understanding of treatment needs; Knowledge of available treatment resources and appropriate use; Population-specific communications skills.

SCHEDULE: Monday - Friday 8:30a-5:00pm

RESPONSIBILITIES: • Oversees a multi-disciplinary team of care managers and peer support specialists to ensure adequate coordination with community resources to meet the unique biopsychosocial needs of individuals served; • Ensure completion of thorough and timely documentation in the electronic care record as well as data collection of key outcomes and performance indicators; • Provide support and oversight of CCBHC including developing and maintaining relationships partner providers in Burlington County including, but not limited to: psychiatric hospitals, substance use residential facilities, acute care settings, Hospital systems, social service agencies and primary care providers; • Utilize clinical and administrative client data from internal and external databases to coordinate services and resources; • Assist with agency use of the Health Information Exchange database to enhance client's transitions of care; • Provide individual supervision and facilitate team meetings per agency standards and best practice; • Navigate services with multiple systems of care, treatment providers and other social service providers according to the needs and preferences of the individual served; • Provide care coordination to consumers by facilitating placements including referral and linkage to treatment programs, providers and community resources as needed; • Ensure completion of thorough and timely documentation in the electronic care record as well as data collection of key outcomes and performance indicators; • Perform program evaluation by reviewing quarterly reports, continuously monitors program outcomes and objectives and communicates with third-party evaluator as necessary and required; • Ensure programs are meeting productivity, Level of Service (LOS) and revenue targets; • Ensure programs are providing services that meet contractual requirements for grant programs; • Participate in committees and attend all meetings as appointed internally and in the community; • Recruiting, training, and developing a highly motivated and effective team; • Provides guidance, leadership and coaching to employees and ensures timely feedback including performance appraisals; • All other duties as assigned.
