QUALIFICATIONS
  • Bachelor’s degree in SW, psychology or related degree with a minimum of 2 years as a Case Manager and 2 years working in the mental health field
  • Some evening hours required
  • Flexible schedule as some duties will take place in the evening hours
  • Driver’s license with acceptable driving history a must
RESPONSIBILITIES
  • Working with and supporting family members and caregivers with an adult relative living with mental illness
  • Provide case management services, including outreaches, linkages, advocacy, referrals, and general support
  • Assist to facilitate multi-family psycho-education and support groups, including researching ongoing psycho-education topics, presentations
  • Provides ongoing and communication with open caseload
  • Ensure proper documentation in a timely and consistent manner
