#26-31 Case Manager HARP / CIS, Full Time, Oahu - 2 positions Case Manager provides professional counseling and case management services for specific client populations. Position involves evaluating and assessing potential clients' needs, linking them to appropriate services and/or assist with placement as needed. Em ployee must be also able to travel to prospective clients' and referral sources sites, conduct regular office/home visits and visits to vendors' business locations. The Case Manager must demonstrate an active commitment to Catholic Charities Mission and st rive to incorporate the four Core Values in all aspects of daily work. EDUCATIONAL & WORK REQUIREMENTS: Must have a Bachelor's degree in human services, social services or related field plus a minimum of three (3) years experience working with the assig ned population. Bachelor's Degree may be substituted with (4) additional years experience working with the assigned population.
